  1.      Thank you for your inquiry into the Wake-up Service.  The Wake-up
  2.      Service is a computerized service which will place wake-up calls to
  3.      your residence at your individual pre-programed times.  The Wake-up
  4.      Service can place wake-up calls anytime, 24 hours per day, 7 days
  5.      per week.  Several wake-up calls to you each day is also optional.

  7.      Your individual wake-up call times are programed into the Wake-up
  8.      System for each selected day of the week.  The Wake-up System will
  9.      then follow this programming each week as it places your daily wake-up
  10.      calls to you.  For each Wake-up call, there will be a charge as
  11.      indicated at the bottom of this form.  Each month, you will receive a
  12.      bill which itemizes the date and exact time of each wake-up call.

  14.      The Wake-up Service is cancellable by you at any time and only
  15.      previously unbilled wake-up calls will subsequently be billed to you.
  16.      Additionally, certain holidays are exempt from providing service.  The
  17.      dates of these holidays are also indicated at the bottom of this form.

  21.      this form.  This service is available only to residences within the
  22.      local calling area of this service.
